THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment to Your Privacy
The Traveling PCP, PLLC ("The Traveling PCP," "we," "us") is required by law to protect the privacy of your health information, give you this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
"Health information" means information about your physical or mental health, the care you receive, or payment for your care that can identify you. This notice applies to all health information we create or receive, including information stored in our telehealth platform.
How We Use and Share Your Health Information
We may use and share your health information without your written authorization for the following purposes.
Treatment: We use your information to evaluate, diagnose, and treat you, and we may share it with others involved in your care. Example: We send your prescription to your pharmacy, or share your records with a specialist we refer you to.
Payment: We use your information to bill and collect payment for our services. We are a self-pay practice and do not bill health insurance. Example: We use your name and card information to process your $110 visit fee, or to respond if you dispute a charge.
Health care operations: We use your information to run our practice and improve care. Example: We review records to check the quality of our care, train staff, or work with our attorneys and accountants.
Business associates: We share information with vendors that help us operate, such as our telehealth platform (Healthie). They must sign agreements to protect your information.
Communications with you: We may contact you by email, text, or app notification about your visits, care plans, and practice updates, as described in your communication consent.
Family and friends involved in your care: We may share information with a family member, friend, or caregiver involved in your care or payment, if you agree or do not object. If you are unable to agree, we may share it if we believe it is in your best interest.
Parents and guardians of minors: We share a minor's health information with the parent or legal guardian authorized to make their medical decisions, as allowed by state law.
Other Uses and Disclosures Allowed by Law
We may also share your health information without your authorization when required or permitted by law, including:
As required by law, such as when federal, state, or local law requires it.
Public health activities, such as reporting certain diseases, adverse reactions to medications, or product recalls.
Abuse, neglect, or domestic violence, including reporting suspected abuse or neglect of a child, elderly person, or person with a disability.
Health oversight, such as audits, investigations, and licensing by agencies like the Board of Nursing.
Legal proceedings, in response to a court order, subpoena, or other lawful process.
Law enforcement, in limited situations, such as to identify a suspect or report a crime.
Serious threats to health or safety, to prevent or lessen a serious and imminent threat to you or others.
Coroners, medical examiners, and funeral directors, as needed to carry out their duties.
Organ and tissue donation, to organizations that handle donation and transplantation.
Research, when approved under strict privacy rules.
Specialized government functions, such as military, national security, or protective services.
Workers' compensation, as allowed by workers' compensation laws.
When Texas or another state's law gives your health information more protection than federal law, we follow the more protective law.
Uses That Require Your Written Authorization
We will not do the following without your written authorization:
Use or share psychotherapy notes, if any exist.
Use or share your information for marketing.
Sell your health information.
Share your information for any other purpose not described in this notice.
We will not electronically disclose your health information to anyone outside of treatment, payment, health care operations, or as otherwise permitted by law without your authorization, as required by Texas law.

You may revoke an authorization at any time by writing to us at hello@thetravelingpcp.com. Revoking it will not affect information we already shared while it was in effect.
Your Rights
To use any of these rights, contact us at hello@thetravelingpcp.com.
